Pledge form

If you complete the form we require your:

  • 280 character pledge.
  • Name.
  • Location information of your place of work.

We will use this information to display your committed pledge on the website. Please note this website is publically accessible so if you do not wish for your pledge to be shown publicly, please do not fill out the form.

The pledge information will be used for staff communications, promotions and marketing within the West Yorkshire Health and Care Partnership.

We will not share this information with any other third party or organisation.

This information will be stored as long as the website and movement is live for.

If you do not wish for us to store these details please do not submit the form.

This website is hosted in a fully segregated hosting environment. Access to the hosting environment is restricted to the Magpie development team.

Access to the content management system data is restricted to the Communications team at West Yorkshire Health and Care Partnership and the development team at Magpie.

You are able to remove yourself from campaign communication and Portal access by contacting us using the information below.
